What You’ll Do:

As an LPN, you’ll play a vital role in supporting our patients and RN Case Managers by:

• Following the individualized plan of care and physician’s orders to deliver safe, high-quality nursing care.

• Teaching patients and families simple self-care techniques to support independence.

• Monitoring patient progress, reporting changes in condition, and suggesting updates to care plans.

• Providing positive, compassionate communication to patients, families, and caregivers.

• Helping prevent unnecessary ER visits or hospitalizations by delivering proactive care at home.

What You Need:

• Graduate of an accredited LPN/LVN program

• Current State License (LPN/LVN)

• Minimum 1 year of clinical experience (home health or hospice a plus)

• Reliable transportation, valid driver’s license, and insurance

• Ability to lift 50–100 lbs as needed
