HealthPro Heritage is excited to present a fantastic PRN Physical Therapist (PT) opportunity within our Senior Living Division. In this role, you will provide outpatient therapy services to adult residents at our assisted living facility located in Charleston, IL. Therapy is delivered onsite in an outpatient clinic setting. Per Diem Wage Range: $50-$58 per hour

Responsibilities:
- Patient Assessment: Evaluate patients' physical conditions including mobility, strength, and flexibility.
- Treatment and Education: Deliver therapeutic exercises, manual therapy, and other treatments to enhance functionality and alleviate pain. Educate patients and families about exercises, posture, and body mechanics to aid recovery and prevent injuries, providing guidance on home routines and lifestyle changes.
- Documentation: Keep accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
- Collaboration: Partner with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as required.
Qualifications:
- Education: Degree in Physical Therapy from an accredited institution.
- Licensure: Valid state licensure as a Physical Therapist or eligibility for licensure.
- Skills: Strong clinical assessment and diagnostic skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills with a patient-centered focus.
- Physical Requirements: Ability to lift and assist patients as necessary. Must be capable of standing for extended periods and performing physical tasks related to therapy.
