Care Advocate Responsibilities:
Build strong, trusting relationships with Medicare members based on empathy, respect, and patience.
Engage with members via inbound and outbound communication-calls, video, texts, and email.
Help members overcome barriers by connecting them to community-based resources (food, housing, transportation, etc.).
Support members with scheduling healthcare appointments and accessing care.
Empower members to become self-advocates in managing their health and wellness.
Collaborate with a fast-paced, startup-minded care team to improve systems and processes based on real-time feedback.
Participate in a rotating schedule including evenings and holidays for urgent or crisis-level situations.
Role is 90% virtual with 2-3 in-person community visits per month.
