Your Role and Responsibilities

• Work collaboratively with an interdisciplinary team to customize treatment plans based on patient needs.
• Establish discharge plans and contingency strategies with team collaboration.
• Facilitate the patient discharge experience effectively.
• Monitor and enhance patient experience, adhering to quality and service standards.
• Lead weekly team conferences, coordinating treatment plan adjustments as needed.
• Complete necessary case management documentation accurately.
• Stay updated with regulations, policies, and operational standards.
• Analyze case management reports and implement action plans based on Key Care Indicators.
• Attend Acute Care Transfer meetings to address trends and improve patient transitions.
• Engage with patients and families according to arrival and visit guidelines within 24 hours of admission.
• Assess goals and complete related documentation within 48 hours post-admission.
• Educate patients and families about rehabilitation and your role as a Case Manager; establish a communication plan.
• Schedule family conferences as necessary.
• Assist patients in ensuring timely resource procurement for smooth discharges.
• Oversee compliance in ordering orthotics and prosthetics.
• Make timely referrals to post-discharge providers and ensure proper documentation.
• Ensure accuracy of discharge information within patient records.
• Participate in utilization reviews, including data collection and trend analysis.
• Be available for case management on-call duties as required.

Qualifications

License or Certification:

• You must be qualified to conduct assessments independently based on your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• An active license is required where necessary for your discipline.
• You should meet eligibility requirements for CCM® or ACM certification upon entering this role, or within two years.
• CCM® or ACM certification is required or must be acquired within two years of taking on the Case Manager II position.

Minimum Qualifications:

• For Nursing, an Associate Degree in Nursing is a must, with RN licensure (BSN preferred). Note that diplomas are acceptable in certain states.
• All other eligible licensed or certified healthcare professionals must hold at least a Bachelor’s degree, with a graduate degree preferred.
• A minimum of two years of rehabilitation experience is preferred.
