Nurse Practitioner Stroke Unit Part-Time

The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.

Responsibilities include performing and documenting accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender, and clinical problem; ordering, performing, and interpreting laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate; managing general medical and surgical conditions based on comprehensive knowledge; prescribing medication safely and effectively; providing care that is patient- and family-centered; adhering to HMH nursing protocols, standards of care, and disease-specific quality indicators; applying evidence-based and value-based medicine; evaluating patient/family transition of care planning; demonstrating clinical expertise through advanced assessment and procedural skills; completing medication reconciliations; participating in annual APN peer review; translating best practice into clinical practice; providing culturally sensitive and educationally appropriate health management education; facilitating consistent, coordinated care and clear communication among all members of the healthcare team; serving as an advocate for patients and families; serving as a clinical resource for the multidisciplinary team; participating in organizational and departmental meetings, committees, and workgroups; demonstrating the highest level of accountability for professional practice; collaborating with medical staff to support nursing-led protocols; mentoring other APNs, precepting students in APN programs; identifying patient/family educational needs; facilitating and participating in nursing educational programs; promoting a collaborative work environment; promoting innovation; participating in clinical research activities; identifying ethical care issues; adhering to the legal framework and scope of practice; complying with HMH and department-specific scope of services; and adhering to HMH organizational competencies and standards of behavior.

Qualifications include a graduate of an NLN/AACN accredited nursing program, a Master's or Doctorate degree from an accredited Nurse Practitioner program, maintaining clinical competencies through continuing education and clinical practice, experience in a relevant patient care setting, a National Provider Identifier (NPI), prescriptive authority through CDS and DEA registration according to New Jersey state law, a collaborative agreement with physicians on staff, excellent written and verbal communication skills, and proficient computer skills.

Compensation: Minimum rate of $82,212.00 annually. HMH is committed to pay equity and transparency for our team members.
