Nurse Practitioner Primary Care

The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Primary care, outpatient only. The site is to be operational Monday - Saturday 8am to 8pm.

Responsibilities include performing and documenting accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender, and clinical problem; ordering, performing, and interpreting laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate; managing general medical and surgical conditions based on comprehensive knowledge; prescribing medication safely and effectively; seeking opportunities to promote Hackensack Meridian Health (HMH) APN practice recognition; providing care that is patient- and family-centered; adhering to HMH nursing protocols, standards of care, and disease-specific quality indicators; applying evidence-based and value-based medicine; evaluating patient/family transition of care planning; demonstrating clinical expertise through advanced assessment and procedural skills; completing medication reconciliations; participating in annual APN peer review; translating best practice into clinical practice; providing culturally sensitive and educationally appropriate health management education to patients and families; facilitating consistent, coordinated care and clear communication; serving as an advocate for patients and families; serving as a clinical resource for the multidisciplinary team; participating in organizational and departmental meetings, committees, and workgroups; demonstrating the highest level of accountability for professional practice; collaborating with medical staff to support nursing-led protocols; mentoring other APNs, precepting students in APN programs and participating in educational programs for nursing as a clinical expert; identifying patient/family educational needs and collaborating to implement appropriate teaching materials; facilitating and participating in nursing educational programs as a clinical expert; promoting a collaborative work environment supporting the organizational mission; promoting innovation by integrating new clinical knowledge into practice; participating in clinical research activities; identifying ethical care issues; adhering to the legal framework and scope of practice defined by the NJ Division of Consumer Affairs; complying with HMH and department-specific scope of services, policies and procedures; and adhering to HMH organizational competencies and standards of behavior.

Qualifications include a graduate of an NLN/AACN accredited nursing program, a Master's or Doctorate degree from an accredited Nurse Practitioner program, maintaining clinical competencies through continuing education and clinical practice, experience in a relevant patient care setting, National Provider Identifier (NPI), prescriptive authority through CDS and DEA registration according to New Jersey state law, a collaborative agreement with physicians on staff, excellent written and verbal communication skills, and proficient computer skills.
