Nurse Practitioner Primary Care Manalapan Part-Time

The Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Part-time Hours: Tuesday- 10am-2pm Thurs- 9-1

Responsibilities include: performing and documenting accurate, complete, and relevant history and physical assessments; ordering, performing, and interpreting laboratory tests and diagnostic studies/procedures; managing general medical and surgical conditions; prescribing medication safely and effectively; seeking opportunities to promote Hackensack Meridian Health (HMH) APN practice recognition; providing care that is patient- and family-centered; adhering to HMH nursing protocols, standards of care, and disease-specific quality indicators; applying evidence-based and value-based medicine; evaluating patient/family transition of care planning; demonstrating clinical expertise; completing medication reconciliations; participating in annual APN peer review; translating best practice into clinical practice; providing culturally sensitive and educationally appropriate health management education; facilitating consistent, coordinated care and clear communication among all members of the healthcare team; serving as an advocate for patients and families; serving as a clinical resource; participating in organizational and departmental meetings; demonstrating the highest level of accountability for professional practice; collaborating with medical staff; mentoring other APNs; identifying patient/family educational needs; facilitating and participating in nursing educational programs; promoting a collaborative work environment; promoting innovation; participating in clinical research activities; identifying ethical care issues; adhering to the legal framework and scope of practice; complying with HMH and department-specific scope of services, policies and procedures; and adhering to HMH organizational competencies and standards of behavior.

Qualifications include: graduate of an NLN/AACN accredited nursing program; master's or doctorate degree from an accredited Nurse Practitioner program; maintaining clinical competencies through continuing education and clinical practice; experience in a relevant patient care setting; national provider identifier (NPI); prescriptive authority through CDS and DEA registration according to New Jersey state law; collaborative agreement with physicians on staff; excellent written and verbal communication skills; and proficient computer skills.
