The SCC ISNP program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The Optum SCC APC provides care to our highest-risk health plan members and is part of an interdisciplinary team. Optum is transforming care delivery with innovative and personalized care. Primary Responsibilities:
- Conduct comprehensive assessments
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation of acute and chronic medical and behavioral conditions
- Communicate and collaborate with the interdisciplinary care team
- Conduct quality advanced illness and advanced care planning conversations
- Provide members and caregivers with counseling and education
- Utilize advanced clinical nursing expertise, knowledge of geriatric/chronic disease management, and the long-term care industry to provide best in class care to members
- Foster and develop a culture of clinical expertise
- Anticipate customer needs and proactively develop solutions to meet them

Required Qualifications:
- Active and unrestricted license in the state which you reside, or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
- Ability to gain a collaborative practice agreement, if applicable in your state
- APCs working in jurisdictions that authorize APCs to practice autonomously or without formal supervision must have obtained approval to practice autonomously or without formal supervision from their licensing board, if applicable. New hires who are eligible and have not applied prior to hire date, must apply to practice autonomously or without formal supervision within 1 month of hire. If not eligible to practice autonomously or without formal supervision at hire, the APC must begin working towards meeting the requirement within 1 month of hire, if applicable, and apply for approval to practice autonomously or without formal supervision within 3 months of becoming eligible
- Education:
- NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- PA: Graduate of an accredited Physician Assistant degree program
- National Board Certification:
- NP: Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family, Adult, Acute, Gerontology, Adult-Gerontology Nurse Practitioner
- PA: Board certified by the National Commission on Certification of Physician Assistants
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
- Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessment
- Driver's license and access to reliable transportation and willingness to travel in between facilities daily
- High level of comfort with Advanced Illness and Advance care planning with ongoing development ability
Preferred Qualifications:
- 1+ years of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including the ability to document medical information with written and electronic medical records
- Flexibility with shifting priorities

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $43.22 to $77.21 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
