Department: BH Integration Shared Services

FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks)

Shift: Days

Pay: $31.61 - $47.41/hour. Pay is dependent on applicant's relevant experience

Responsibilities:

• Provide effective counseling and therapy to patients in individual and group settings.
• Conduct thorough interviews and assessments involving patients, families, caregivers, or legal representatives.
• Identify and coordinate necessary internal and external services and interventions to support patient care.
• Participate in case reviews and care conferences to assess patient progress.
• Assist with discharge planning by identifying community resources and providing referrals as needed.
• Utilize suitable teaching methods and referrals to meet the educational needs of patients and their families.

Minimum Requirements:

• Master's Degree.
• State licensure as a Licensed Professional Counselor Candidate (LPC-C), Marriage and Family Therapist Candidate (MFT-C), Social Work Candidate (SWC), or Licensed Social Worker (LSW).
