Job Summary: As a Licensed Practical Nurse (LPN) at our healthcare team, you will play a crucial role in delivering comprehensive nursing care to our residents. You will work under the supervision of registered nurses and other healthcare professionals to ensure the well-being and comfort of our residents. Your dedication, clinical expertise, and compassion will directly impact the lives of those in your care.
Responsibilities:
- Perform routine nursing tasks under the guidance of registered nurses, physicians, and other healthcare professionals.
- Administer medications and treatments as prescribed by the healthcare team.
- Monitor and assess residents vital signs and condition, reporting any changes promptly.
- Assist with resident admissions, transfers, and discharges, ensuring proper documentation and communication.
- Provide wound care, injections, and other medical procedures as needed.
- Collaborate with the interdisciplinary team to develop and update individualized care plans for residents.
- Assist in maintaining a clean and safe environment for residents and staff.
- Document all nursing care provided accurately and in a timely manner.
- Educate residents and their families on healthcare procedures and provide emotional support.
- Respond to emergencies and provide first aid when necessary.
- Adhere to all state and federal regulations, as well as facility policies and procedures.
- Participate in ongoing education and training to maintain licensure and stay current with best practices in nursing.
- Valid Licensed Practical Nurse (LPN) license in state of practice.
- CPR certification required.
- Previous experience in a skilled nursing facility or long-term care setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work as part of a team and independently as needed.

- Basic computer skills for electronic health record documentation.
