Licensed Practical Nurse (LPN)
Summary:
Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides direct patient care to residents in a skilled nursing facility and rehabilitation center. The LPN is responsible for administering medications, monitoring patient conditions, and assisting with activities of daily living.
Essential Duties and Responsibilities:
- Provides direct patient care, including but not limited to:
- Administering medications and treatments as ordered by physicians
- Monitoring patient conditions and vital signs
- Assisting with activities of daily living, such as bathing, dressing, and grooming
- Performing wound care and other treatments as prescribed
- Collecting and documenting patient data
- Collaborates with the interdisciplinary team to develop and implement patient care plans
- Participates in patient education and discharge planning
- Maintains accurate and up-to-date patient records
- Adheres to all facility policies and procedures, including infection control and safety standards
- Assists with resident admissions, transfers, and discharges
- Participates in quality improvement activities
- Performs other duties as assigned
- Current LPN licensure in the state of Ohio
- Basic Life Support (BLS)
- Experience in long-term care or rehabilitation preferred
- Strong clinical skills and ability to prioritize tasks
- Excellent communication and interpersonal skills
- Ability to work effectively as part of a team
- Ability to lift and transfer patients
- Ability to stand, walk, and sit for extended periods of time
- Ability to lift and transfer patients
- Ability to perform tasks requiring manual dexterity
