Job Summary

As a Licensed Practical Nurse (LPN) in our Long Term Care setting, you will play a vital role in delivering professional nursing care for residents of all ages. You will work under the supervision of a registered nurse or physician, demonstrating your knowledge of fundamental nursing principles tailored to the resident's specific care plan.

Key responsibilities include:

• Ensuring the health, comfort, and safety of residents.
• Contributing to resident assessments and administering medications or treatments as prescribed.
• Documenting interactions related to resident care and behavior.
• Building effective working relationships with residents, families, and the healthcare team.
• Communicating clearly and maintaining confidentiality of sensitive information.
• Counseling residents and family members on prevention and treatment plans under appropriate supervision.
• Performing additional duties as assigned.

Qualifications

To be considered for this role, you must:

• Be a graduate from an accredited practical nursing program and hold a current Licensed Practical Nurse (LPN) license with the relevant state nursing board.
• Have experience in a pre-licensure nursing program that included supervised clinical practice across the lifespan or have a minimum of 400 hours of experience as an LPN at another facility.
• Maintain required competencies and certifications.
• Complete annual continuing education units (CEU) and training as necessary.
