Your Responsibilities:

• Provide in-home direct patient care per the prescribed plan of care while effectively monitoring patient progress.
• Assess, document, and report changes in patient conditions (including vitals, blood/stool/urine, glucose, etc.).
• Administer medications, insulin, and IV/fluids, ensuring thorough documentation.
• Care for wounds, change dressings, and assist with personal hygiene needs.
• Evaluate patients for potential injuries, infections, or bedsores and implement appropriate treatments.
• Collaborate with healthcare providers and allied health professionals to ensure outstanding patient outcomes.

Qualifications:

• Education: Graduate of an accredited practical nursing program with a current LPN license in the state of employment.
• Licensure: Active and unrestricted LPN license.
• Certification: Current CPR/AED/BLS/First Aid certification.
• Transportation: Reliable transportation for travel to various care sites.
• Experience: Practical trach and/or ventilator experience is preferred but not required.
