Licensed Practical Nurse - Hospice Care

As a Hospice LPN, you will:

• Directly report to the Administrator or Executive Director.
• Provide essential patient care services and emotional support to patients and their families.
• Assess patient and family needs to assist in developing and implementing personalized care plans.
• Communicate any changes in the patient’s condition to the Registered Nurse and the interdisciplinary team, ensuring accurate documentation.
• Participate actively in staff meetings to contribute to team dynamics.

About You:

• A compassionate individual, displaying kindness and empathy towards patients and families.
• Comfortable delivering care in various settings, ensuring dignity and respect.
• Flexibility and composure, especially during times of crisis.
• Current LPN license in the state of employment.
• Minimum of 1 year of relevant patient care experience, particularly in hospice, home health, ICU, geriatrics, or oncology is highly valued.
• Preferred experience may include roles in Med Surg, ER, critical care, wound care, and similar environments.
• Valid driver's license and automobile insurance, along with current CPR certification.
