Job Summary

The Licensed Practical Nurse (LPN) is responsible for implementing and evaluating patient care for stable patients of all ages in the home care setting. You will demonstrate nursing knowledge relevant to patient care, conduct assessments, and provide necessary care based on age-specific needs. You will ensure patient health, comfort, and safety while maintaining confidentiality and effective communication.

Key Responsibilities:

• Administer treatments and medications as directed.
• Document and maintain accurate patient records.
• Establish effective relationships with patients, healthcare providers, and the public.
• Address patient problems and provide appropriate solutions.
• Communicate clearly and maintain confidentiality.
• Counsel patients and families about treatment and prevention plans under RN guidance.

Qualifications

• Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
• One year of professional nursing experience preferred.
• Currently holds an unencumbered LPN license with State Nursing board and/or multi-state licensure.
• Required competencies and certifications for the department.
• A valid driver's license may be needed depending on location.
