Your Role as Case Management Director

In this pivotal role, you will:

• Manage the daily operations and human resource functions of the Case Management department.
• Oversee the interdisciplinary plan of care and discharge planning processes, focusing on patient care outcomes and census management.
• Advocate for patients and their families, ensuring their needs are met through high-quality services.
• Provide guidance and support to Case Managers and staff, offering training on case management processes.
• Build and maintain relationships with insurance companies, self-insured employers, and other healthcare networks.
• Celebrate the achievements of our dedicated staff and foster a culture of recognition.

Qualifications for Success

• Current CCM® or ACMTM certification preferred.
• Ability to complete assessments independently within your scope of practice.
• If required by state law, maintain an active license in your discipline.
• Nursing candidates must hold a Bachelor’s degree in Nursing (BSN) and RN licensure.
• Other healthcare professionals should have a minimum Bachelor’s degree; a graduate degree is preferred.
• At least three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• Willingness to work flexible hours, including nights, weekends, and holidays as needed.
