Salary: $100,000.00 - $130,000.00 per year
Essential Duties and Responsibilities:

Conduct thorough medical histories, perform physical examinations, and prescribe diagnostic tests to establish customized care plans.
Collaborate with physicians and other medical professionals to provide diagnosing and treatment services.
Develop and monitor individualized patient care plans in correlation with medical protocols.
Partake in a collaborative clinical teaching environment, enhancing both patient care and professional education.
Maintain regular communication with patients, ensuring clarity in treatment plans and healthcare advice.
Perform responsibilities with an understanding of patient confidentiality and medical ethics.
Utilize Epic Electronic Medical Record system to maintain accurate patient records.
Adapt to a flexible schedule that includes standard working hours, occasional Saturday mornings, and potential early morning or early evening meetings to optimize patient service delivery.

Minimum Position Qualifications:

NYS licensed/registered Nurse Practitioner with a specialty area of certification aligned with family practice.
Valid National Provider Identification (NPI) number and either current or eligibility for Medicare Enrollment within 6 months of hire.
Current CPR certification through American Heart Association or American Red Cross.

Preferred Qualifications:

3-5 years of experience as a Nurse Practitioner in a primary care or outpatient setting.

Skills:

Strong ability to communicate effectively in both oral and written formats with patients, families, and other health professionals.
Proficient in using healthcare database software, including Electronic Medical Record systems like Epic.
Excellent clinical skills with ability to perform comprehensive physical examinations and necessary diagnostic procedures.
Demonstrated ability to work autonomously as well as part of a team in a medically diverse environment.
Strong organizational and time management skills to ensure timely and accurate patient care.
