We are looking for a passionate and experienced Hospice Clinical Liaison to join our exceptional team. In this crucial role, you will facilitate intake and referral processes with efficiency and empathy to meet the needs of our patients, families, and healthcare partners. Your dedication will play a key role in nurturing strong professional relationships, managing admissions, and enhancing community awareness of our hospice services.

Key Responsibilities

• Promptly respond to referrals from various healthcare professionals including hospitals, home health agencies, physicians, and skilled nursing facilities.
• Assess patient eligibility for hospice care through comprehensive medical record reviews and clinical evaluations while communicating effectively with involved physicians and families.
• Provide education to patients and families about hospice care, ensuring they understand its value and how the process works, always with compassion and clarity.
• Complete all required documentation to facilitate the intake and admission process, meeting all legal, clinical, and payer criteria.
• Act as a liaison between referral sources and our hospice care team, ensuring seamless communication and rapid response to all needs.
• Make regular visits to hospitals and facilities to foster visibility and collaboration with discharge planners, case managers, and physicians.
• Develop and nurture strong relationships with referral partners while identifying new outreach opportunities to increase community awareness about hospice services.

About You

Education

• Licensed Nurse with three to five years of hospice experience is required.
• BSN with five or more years of hospice experience is preferred.

Experience

• A minimum of three years of hospice experience is required.
• Prior experience as a discharge planner, case manager, or educator is preferred.

Licenses and Certifications

• Current RN license to practice nursing in your state.
• CPR certification.
• Valid driver's license and automobile insurance.

Skills and Knowledge

• Strong understanding of hospice care and the specific needs of terminally ill patients and their families.
• Familiarity with community resources and referral networks.
• Exceptional patient assessment and documentation skills.
• Well-versed in staff supervision, service coordination, and effective program management.
• Excellent communication and interpersonal skills, with sensitivity to diverse backgrounds.

Compensation may vary within the salary range based on factors such as location, experience, education, skills, licensure, certifications, and department equity. Gentiva offers a comprehensive benefits package, with base pay being just one component.

Salary Range - $90,000 to $95,000
