Hospice Clinical Coordinator

In this vital role, you will facilitate efficient and effective intake and referral processes, ensuring the needs of patients, families, referral sources, and our organization are met.

Key Responsibilities:

• Actively engage with referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals.
• Assess patient eligibility for hospice services through comprehensive reviews of medical records, clinical evaluations, and effective communication with physicians and families.
• Educate patients and families about hospice care, providing clear information, reassurance, and compassionate support.
• Oversee the documentation and coordination of the intake and admission process, ensuring compliance with legal, clinical, and payer requirements.
• Serve as a bridge between referral sources and our hospice care team, promoting seamless transitions and open communication.
• Conduct regular visits to hospitals and facilities to maintain visibility and nurture relationships with discharge planners, case managers, and physicians.
• Cultivate and sustain strong relationships with referral partners, while identifying new outreach opportunities to enhance community awareness of hospice services.

Your Qualifications:

• Education: Licensed Nurse with 3-5 years of hospice experience required; BSN with 5+ years of hospice experience preferred.
• Experience: Minimum 3 years of hospice experience required; prior experience as a discharge planner, case manager, or educator preferred.
• Licenses and Certifications: Current RN license in the state of employment, CPR certification, and valid driver's license with automobile insurance.
