As a Revisit LPN, you will leverage your education and skills to provide personalized and rewarding care to our patients.

Key Responsibilities:

• Facilitate communication between the patient and the interdisciplinary care team.
• Build and maintain therapeutic relationships while educating patients and promoting their self-advocacy and independence.
• Document the skilled services provided using state-of-the-art technology.

Qualifications:

• Valid CT LPN license required.
• One year of LPN experience is essential; home care experience is preferred.
• A valid driver's license and reliable transportation are necessary.
• Proficiency in computer skills is required.
