Assisting Hands (Hudson) is looking for an experienced RN with a valid license in New Jersey and a minimum of 1 year of Home Care experience.
Your responsibilities include:

Conducting thorough client assessments and re-assessments
Formulating comprehensive care plans
Performing 30-day check-in and 60-day care monitoring visits
Conducting supervisory visits
Regular communication with patients, families, caregivers and care-stakeholders.

Qualifications:

Valid nursing license in State of New Jersey
One year experience in Home Care highly preferred.
Has a valid driver's license and a reliable mode of transportation.
Able to travel to clients' homes throughout Hudson and Southeast Bergen counties.
Strong communication and interpersonal skills
Professional Liability Insurance 1M - 3M(Preferred)
