Job Responsibilities and Duties:

• Conduct comprehensive patient exams and review test results

• Diagnose and manage acute and chronic conditions

• Document patient encounters using EMR templates within 72 hours

• Educate patients on care plans, medications, and lifestyle changes

• Coordinate follow-up resources and referrals

• Participate in clinical huddles and collaborate with the care team

• Consult with physicians when appropriate

• Advocate for access to care and use of community resources

• Set and monitor patient self-management plans

• Engage ancillary health team members in patient management

• Ensure continuous, coordinated, and community-focused care
