• Provide virtual support for patients undergoing Type 2 Diabetes management programs
• Oversee weight management programs
• Manage patient panels on GLP-1 medications
• Utilize experience in chronic disease management and population health
• Collaborate with the healthcare team to ensure comprehensive patient care
• Maintain accurate and up-to-date patient records using Athenahealth EMR system
• Adhere to all relevant healthcare regulations and best practices
