Your Role as Director of Case Management

1. Manage the daily operations and human resources of the Case Management department.
2. Oversee the interdisciplinary care plan and discharge process to enhance service effectiveness, focusing on patient outcomes and key care indicators.
3. Advocate for patients and families, ensuring their needs are met through our services.
4. Guide and support Case Managers and staff with training on caseload management and compliance with regulations, policies, and objectives.
5. Establish and maintain relationships with insurance companies, employers, and other healthcare networks.
6. Recognize and celebrate the successes of our dedicated employees.

Qualifications

• CCM® or ACMTM certification is preferred.
• Must be qualified to conduct assessments within your discipline.
• Active licensure is required where applicable.
• If a nurse, must possess a bachelor’s degree in nursing (BSN) with active RN licensure.
• Other healthcare professionals should have a minimum of a bachelor’s degree; graduate degrees are preferred.
• A minimum of three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.
• Willingness to work various shifts, including weekdays, weekends, evenings/nights, and holidays as required.
