Become the Director of Case Management Services you've always aspired to be

• Oversee the day-to-day operations and human resource management of the Case Management department.
• Manage the interdisciplinary care plan and discharge process, ensuring effective and suitable services focused on patient care outcomes and key care indicators.
• Act as an advocate for patients and their families, ensuring services meet their needs.
• Provide guidance and support to Case Managers and staff, offering training on caseload management, regulations, policies, and operational procedures.
• Build relationships with insurance companies, case management firms, self-insured employers, and other healthcare networks.
• Celebrate the achievements and successes of our dedicated staff.

Qualifications

• Current CCM® or ACMTM certification is preferred.
• Must be qualified to conduct assessments within the scope of practice of your discipline.
• If licensure is required in your field, you must hold an active license.
• For Nursing, a bachelor's degree in nursing (BSN) with RN licensure is required.
• Other eligible healthcare professionals should possess at least a bachelor's degree; a graduate degree is preferred.
• A minimum of three years of experience in hospital-based Case Management, including Utilization Review and Discharge Planning.
• Availability to work weekdays, weekends, evenings, and/or night shifts may be required.
• Willingness to work on religious or legal holidays as scheduled may be needed.
