Compassionate Hospice LPN

We are seeking a dedicated and empathetic Hospice Licensed Practical Nurse (LPN) to become a vital part of our hospice team. Reporting directly to the Administrator or Executive Director, you will deliver hands-on care to patients and families in their homes, assisted living, or facility settings, providing comfort during the most critical moments in life.

Key Responsibilities:

• Deliver skilled nursing care to hospice patients, ensuring compassionate support for patients and their families.
• Work collaboratively with the interdisciplinary team to assess patient and family needs, contributing to personalized care plans.
• Quickly report any changes in patient conditions to the Registered Nurse and care team for immediate attention.
• Provide empathetic care aligned with individual plans for each patient.
• Act as a trusted resource for families, offering comfort and guidance during difficult times.

Core Duties Include:

• Administer direct nursing care under the supervision of a Registered Nurse, adhering to established protocols and the Nurse Practice Act.
• Continuously monitor patients, documenting changes in their condition and reporting as necessary.
• Administer medications and treatments as outlined in the patient's care plan.
• Participate in infection control practices and uphold standard precautions.
• Assist with bereavement activities and provide support for families.
• Be available for on-call duties, responding to patient needs during scheduled visits or emergencies.
• Collaborate with hospice aides and other team members to ensure comprehensive care delivery.

Qualifications:

• Graduate from an accredited Practical Nursing program.
• Active LPN/LVN license in the state of employment.
• Minimum of 2 years of LPN experience in hospice, oncology, long-term care, or home health; 3-5 years preferred.
• CPR certification is required.
• Must have a valid driver's license and current automobile insurance.

Skills and Knowledge:

• In-depth understanding of hospice care principles and the unique needs of terminally ill patients and their families.
• Strong patient assessment and clinical skills.
• Excellent communication and documentation abilities.
• Familiarity with regulatory guidelines including Medicare, Medicaid, CMS, and JCAHO standards.
• Ability to work independently and collaboratively as part of an interdisciplinary team.
