Care Transitions Coordinator

This position provides certified home health and hospice care coordination and discharge planning for patients referred by the hospital's case managers to support the delivery of effective and efficient home health and hospice care. Collaborates with members of the health care team to identify appropriate utilization of resources and to ensure maximum reimbursement. Serves as a liaison with patients and the health care team to ensure a smooth transition from hospital to home health and/or hospice care. Maintains knowledge of current laws and regulations of federal, state, and regulatory bodies relative to home health and hospice and works within those constraints to maintain actions within the agency's and company's compliance programs.

Major responsibilities include providing initial introduction to the patient/family, completing initial screening of all referred patients, utilizing a combination of clinical information to evaluate patients for appropriateness of admission type, collaborating with third party payers to ensure appropriate reimbursement, identifying potential barriers to discharge or transfer, consulting with medical staff, identifying cases that fail to meet established criteria, facilitating a patient-specific, safe discharge plan, contacting patients after discharge, visiting and establishing effective business relationships with physicians, and delivering and participating in health educational services.

Standard staffing level responsibilities include complying with departmental policies, attending meetings, demonstrating use of Quality Improvement, complying with health and safety regulations, respecting diverse views, maintaining regular attendance, and performing other similar and related duties.

Position qualifications include completion of an accredited Licensed Practical Nurse Program, current licensure as a Licensed Practical Nurse in Massachusetts, minimum five years as a Clinical Liaison or similar role predominantly in hospice and certified home health, ability to build and maintain effective relationships, and strong verbal and written communication skills.
