SUMMARY:

In this role, you'll provide high-quality, individualized patient care while working closely with our nursing team to ensure that every resident's needs are met. Your proactive approach to patient care will be crucial in fostering a positive environment for both residents and staff.

RESPONSIBILITIES:

• Patient Care & Service: Assist residents with daily activities as per individualized care plans, transport them to and from activities, provide leisure activities, perform evaluations and reporting, and offer emotional support to both residents and families.

• Safety: Help maintain a clean and safe environment for residents, ensuring their well-being at all times.

• Communication & Documentation: Serve as a liaison for families, guiding them to appropriate staff, responding promptly to resident requests, and effectively communicating with team members during report exchange.

REQUIRED QUALIFICATIONS:

• Valid Certified Nursing Assistant certification in New York State.

PREFERRED QUALIFICATIONS:

• High School Diploma or equivalent.

• Strong customer service and communication skills.

EDUCATION:

LICENSES / CERTIFICATIONS:

• CNA - Certified Nursing Assistant - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

Medium Work - This role may involve exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects. Frequent walking, standing, or squatting is required.

PAY RANGE:

$19.90 - $21.10

The listed base pay range is a good faith representation of current potential base pay for a successful full-time applicant. Compensation may be adjusted in the future and is eligible for additional pay components based on factors such as experience, qualifications, specialty, internal equity, location, and contractual obligations.
