Your Role as a Case Manager:

• Collaborate with an interdisciplinary team to guide patient treatment plans based on individual needs.
• Coordinate discharge planning and facilitate team conferences weekly.
• Ensure timely patient experience and actively monitor services throughout rehabilitation.
• Participate in case management documentation and maintain regulatory knowledge.
• Assist patients with timely resource procurement to avoid discharge delays.
• Educate patients and families on the rehabilitation process and establish communication plans.

Required Qualifications:

• A qualified healthcare professional (RN, SW, OT, PT, ST, or Rehabilitation Counseling) with the ability to complete assessments within your discipline.
• An active state license is required if licensure is necessary for your discipline.
• Current eligibility for CCM® or ACM certification upon entry, or within two years of starting the position.
• Strong foundation in rehabilitation with a preference for at least 2 years of relevant experience.
