Become the Case Management Specialist You Aspire to Be

• Collaborate with an interdisciplinary team to guide individualized treatment plans.
• Coordinate care plans and prepare for a successful patient discharge experience.
• Monitor the quality and appropriateness of patient experiences throughout their treatment.
• Facilitate team meetings and manage ongoing treatment adjustments.
• Keep up-to-date with case management reports and implement necessary actions.
• Educate patients and their families on rehabilitation processes and the Case Manager's role.
• Ensure timely referrals and proper documentation for post-discharge care providers.
• Participate in the utilization review process to analyze care trends.
• Be part of the on-call schedule as needed.

Required Qualifications

• Must be qualified to conduct assessments within the scope of your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• Active licensure as required by your discipline.
• Eligibility for CCM or ACM certification within two years of entering the role.
• For nurses, an Associate Degree in Nursing with an RN license is required; BSN preferred.
• For other professionals, a minimum of a bachelor's degree is required, with a graduate degree preferred.
• Two years of rehabilitation experience is preferred.
