Job Summary

Your role as a Behavioral Health Clinician involves collaborating with interdisciplinary teams to deliver evidence-based clinical services throughout the continuum of care. You will uphold ethical standards and professional practices in accordance with our discipline's code of ethics.

Key Responsibilities

1. Conduct comprehensive bio-psychosocial assessments and develop tailored care plans that promote optimal clinical outcomes.
2. Engage with interdisciplinary teams to evaluate appropriate levels of care and implement effective discharge plans.
3. Apply specific therapeutic methods and monitor patient progress, adjusting care plans as necessary.
4. Provide education to patients and families regarding disease prevention, health maintenance, and treatment adherence.
5. Deliver direct patient care, including case management, crisis intervention, and emotional support for individuals facing health challenges.
6. Facilitate referrals to resources and levels of care that align with individualized treatment plans.
7. Maintain accurate, timely documentation of patient assessments, interventions, and outcomes, in compliance with regulations.
8. Participate in team meetings, contribute to staff education, and support the development of programming to meet patient needs.
9. Collaborate with colleagues to enhance the patient experience and ensure the effective use of resources.

Qualifications

• Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or a related human service field.
• Previous experience in a healthcare setting.
• Possession of LMSW, LPC-A, LMFT-A, or CADC certification.
• Valid Driver's License.
• Proficient in computer skills.
• Ability to assess issues, formulate solutions, and make timely decisions.
• Willingness to use team approaches and supervision in patient assessment and intervention.
• Exceptional communication skills capable of fostering relationships within the healthcare system and community.
• Awareness of health-related social needs and cultural competence, addressing disparities in access and service provision.
• Commitment to cultural competence, respecting diverse client histories and values in healthcare interactions.
• Proactive in self-improvement, seeking resources and training to maintain licensure and enhance skills.
